The entry form can be printed, completed and brought with you at the time you enter your photo(s). Entry Forms will also be available at each registration date held at the Perkins Township Service Facility located at 2610 Columbus Ave, Sandusky. Registration dates are: Sept 9th, 2:30pm-4:30 pm; Sept 10th, 5:30pm-7:30 pm; Sept 11th, 11 am-1:00 pm. There is a $1.00 entry fee per photo.
